How do I enroll?

How To Enroll

If you want to enroll your child in the Children's Buy-In Program so they can become a CareSource member, you must first complete the application process through a secured website.

Parents or legal guardians may complete an application for their children as long as they live in the same household. You will be required to have an active e-mail account to complete the application process.

If your child is enrolled in the CBI program, you will be required to pay a monthly insurance premium. The amount of your premium depends on the size of your family and your household income (click here to view CBI premium and cost sharing information). Once your child is found eligible, a billing statement will be mailed to you each month indicating the premium amount. An annual deductible, co-payments and coinsurances are also required for each child.

If you have any questions about the CBI application or about the program in general, please call the Ohio Department of Job and Family Services (ODJFS) toll-free phone number at 1-877-872-8042.
